The reality star lost her grandmother, Mary Stotz, and took to Instagram on Tuesday to share throwback pictures and memories in her honor.

"Goodbye Mommom. You will forever be in my heart," the 30-year-old captioned one photo.

Another touching photo shows the model's grandmother by her side while Wilkinson rests in a hospital bed.

After thanking fans for their condolences on Twitter, the former Playboy model admitted, "I'm still in this dream like state. The world stopped. Nothing makes sense."
